And now - attention! The next group of questions requires special attention, because it is related to your health, and the lack of information can put an end to your trip.
It is "Did you get tested for COVID-19 within 48 hours before your flight?". Answer "Yes" or "No". Required is marked with a checkmark.
But that's not all. You need to indicate the name of the medical organization that conducted the test, as well as the date of the test, its type and test result ("Positive" or "Negative", mark the appropriate one).
But keep in mind: filling in the test data in the questionnaire is not enough, you need to have copies of the certificates with you.
Keep in mind that you may need copies of such certificates abroad, so just in case, make them with a margin.
And the last thing you need is to enter your first name, last name and patronymic in the following text:
